Did You Know In an effort to recover millions of dollars in unpaid fines owed to the province, the Ministry of Justice and Attorney General began partnering with the Canada Revenue Agency in the 2008-09 tax season to garnish federal government income tax and GST credits from people who have not paid fines. Who can I consult? Your family physician can discuss the findings with you and give you information on the medical history of your family. The Chief Coroner can be consulted by written request. My insurance company requires me to complete a "Proof of Death" and "Physicians Statement of Death" form for my claim. How do I complete this? If a coroner is involved in the death enquiry, forward the form and your written request to the Chief Coroner. Most insurance companies require advice on the medical cause of death and the manner of death, so the Chief Coroner must wait until the coroner's enquiry is finished before the form can be completed. Once the forms are completed, they can be forwarded directly to the insurance company or returned to the family. |
